ARHN Online Community Health Assessment Data

The Online Community Health Assessment (CHA) data for the seven County members of the Adirondack Rural Health Network (ARHN) are organized around ten priority areas for improved health. The majority of the data are compiled from publicly available NYS DOH health indicator data sets. The NYS DOH website provides a Statistics & Data webpage where the majority of their public health data resources are summarized. Links to specific NYS DOH data tables are provided throughout this website.

The ten priority health areas are listed below and each is linked to the page where that ARHN CHA data are displayed for each of the seven counties.

This compilation of Community Health Assessment data is maintained by the Adirondack Rural Health Network for its partners and collaborators, with the assistance of Holmes & Associates.  More information about the Adirondack Rural Health Network can be found at:  http://www.arhn.org.

There are three main goals for this regional online CHA data effort:

  1. To establish an online repository of Community Health Assessment data for the counties, hospitals and other organizations participating in the ARHN regional CHA program.
  2. To assist County Health Departments and area Hospitals in identifying their health issues of most concern, while also illuminating the health issues of regional concern.
  3. To facilitate cost-efficient updates and enhancement of the ARHN CHA data.

The latest publication of regional CHA Data for the ARHN area was published in September, 2009 and is entitled Building a Healthy Community.  It was designed around the Prevention Agenda Toward the Healthiest State rationale that was released in 2008 by the New York State Health Commissioner.  The Prevention Agenda is a call to action to local health departments, health care providers, health plans, schools, employers and businesses to collaborate at the community level to improve the health status of New Yorkers through increased emphasis on prevention.

Since publication of the 2009 CHA report, the ARHN region has seen many new initiatives and programs aimed at improving the health of communities by increasing physical activity and nutrition as well as improving access to quality health care. Community gardens are growing, more towns are adopting complete streets policies, health care organizations are continuing their diligent efforts to provide quality health care to all, and work continues to foster the homegrown pipeline that has developed to plan for the future health care workforce in the ARHN region.ARHN-Community Health Assessment Plan-2011

In 2011, ARHN completed a 2011 CHA Update Report to identify how public health departments, hospitals, not-for-profit groups, local governments, churches, schools, and concerned community members are working together for improved health in our communities.
